Photo Release Form

Parenting is tough and we all do it just a little different, but one thing we definitely all have in common is that we LOVE taking pictures of our kids! Here’s an opportunity to promote those great kids in your lives by showing them off on Face It’s social media and blog.  

In order to use the photos, we must have permission from either the person in the picture if he/she is over 18 or a parent or guardian if the picture is of a minor. Fill out the electronic release form below and upload your pictures.

 

By filling out this form and clicking Submit, I, the undersigned, do hereby grant permission to Face It to publicly use photograph(s) of myself and/or my minor child(ren). I understand that the images may be used in print publications, online publications, presentations, websites, videos, and social media. I also understand that no royalty, fee, or other compensation shall become payable to me by reason of such use.
